19ХХ г., в городе Дюссельдорф (Германия), выдающийся доктор Кендзо Тэнма вопреки указу спасает детей политических деятелей, Либерт'ов, убитых в собственном доме. Девочка Анна получила огромный шок, мальчишка Йохан в критическом состоянии. По приказу, Тэнма должен оставить мальчика и оперировать бургомистра. Но… человеческая жизнь не та вещь, которой можно размениваться. Нельзя безнаказанно решать кому жить, а кому умереть. Выгода, полученная за "роль Бога", не станет равноценным обменом или удобной валютой.
После операции дети исчезли. Но по прошествии девяти лет, мальчик вернулся мужчиной, человеком, не считающимся с жизнью. Монстром, оперирующим человеческими жизнями в "благих" целях.
Убивая людей в своих поисках, Йохан пытается найти сестру, с которой расстался в прошлом. Анна, а теперь Нина, живёт, не помня прошлого. Но настигшие воспоминания принуждают её закончить ошибку прошлого.
